Register for the grade your player will be in during the Fall of 2026.
You will need a copy of your players birth certificates as well as a copy of a utility bill to register
Your player must be registered to pick up equipment

Registration will be done through CYFA

Please download and fill out all of the GMSAA forms below. Links to forms are at the bottom of this page.
The GMSAA Waiver, GMSAA COVID Waiver and GMSAA Concussion Awareness Form will be needed to register.
You will need a copy of your players birth certificate as well as a copy of a utility bill to register
You player will also need a physical form on file prior to being able to practice. Physicals are good for 1 year from completion. Please ensure your players Physical form will not "expire" prior to December 2026, otherwise a new physical form will be needed.
